Territory: Bloomington | Eden Prairie | Chanhassen, MN

Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.

Estimated Annual Compensation: $87,000 – $102,000

Compensation Note: This position is paid on a per-point basis. The compensation range reflected in this posting represents estimated annual earnings.

Are you an RN who enjoys building meaningful patient relationships, utilizing strong clinical judgment, and making a difference through individualized care? We are seeking a skilled Home Health Registered Nurse to provide comprehensive nursing services to patients throughout the Bloomington, Eden Prairie, and Chanhassen communities.

Home health nursing is much more than routine skilled nursing. It requires clinical expertise, critical thinking, and the ability to independently manage patients with a wide range of medical needs and acuity levels. You'll have the opportunity to apply your nursing skills while helping patients manage complex conditions, recover, and maintain their quality of life.

What You'll Do:

• Perform comprehensive patient assessments and develop individualized plans of care.

• Manage chronic conditions, including CHF, COPD, diabetes, and hypertension.

• Provide medication reconciliation, management, and patient/caregiver education.

• Perform venipuncture, IV therapy, PICC and central line care, and laboratory specimen collection.

• Manage complex wounds, surgical sites, pressure injuries, and wound VAC therapy.

• Provide Foley and suprapubic catheter care, ostomy care, and drain management, including PleurX systems.

• Manage G-tubes, enteral feedings, and other specialized treatments.

• Provide advanced respiratory care, including tracheostomy care, suctioning, and ventilator management when applicable.

• Coordinate care with physicians, patients, caregivers, and interdisciplinary teams.

• Complete accurate electronic clinical documentation and required assessments.

What We're Looking For:

• Active Registered Nurse (RN) license.

• Previous nursing experience in home health, acute care, med-surg, step-down, LTACH, or critical care.

• Strong clinical assessment skills and sound independent clinical judgment.

• Experience managing wounds, medications, chronic conditions, and medically complex patients.

• Excellent patient education, communication, and care coordination skills.

• Comfortable working independently and making clinical decisions in a home-based setting.

• Proficiency with electronic medical records and clinical documentation.
