The Adult Case Manager provides assessment, linkage, and advocacy services to individuals with developmental disabilities. The role develops and implements individual service plans, completes required documentation, and complies with regulations applicable to program services. Candidates must meet one of two paths: an approved bachelor's degree in a human services field or a valid Virginia registered nurse license, and must have a valid driver's license with a safe driving record.

Full job description

RESPONSIBILITIES Provides assessment, linkage, and advocacy services to individuals with developmental disability; develops and implements individual service plans; completes required documentation; and complies with all regulations applicable to program services. REQUIREMENTS Minimum Requirements M1:Bachelors Degree in a Human Services field (degree subject to approval) OR Registered Nurse with a valid Registered Nurse license to practice in Virginia M2:Valid drivers license with a safe driving record Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
