Serenity Hospice Care is seeking an experienced, compassionate Registered Nurse (RN) to join our team as a Hospice Clinical Care Liaison serving Bergen and Hudson Counties. This individual will work closely with the territory's Hospice Marketer to support referral development, strengthen relationships with healthcare providers, and facilitate appropriate hospice referrals and transitions of care.
The ideal candidate will combine strong clinical knowledge of hospice eligibility and patient needs with excellent communication, relationship-building, and collaboration skills. This position serves as a clinical resource to referral partners, helping them understand hospice services, identify patients who may benefit from hospice care, and coordinate timely referrals in collaboration with the admissions and clinical teams.
Key Responsibilities

Clinical Referral Support: Partner with the territory marketer to identify, develop, and support hospice referral opportunities throughout Bergen and Hudson Counties.

Hospice Education: Educate physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources about hospice eligibility, services, and the benefits of comfort-focused care.

Referral Relationship Management: Build and maintain strong professional relationships with referral partners by providing clinical guidance, timely communication, and responsive follow-up.

Patient Identification: Help referral sources recognize patients who may meet hospice eligibility criteria based on their clinical condition, prognosis, and goals of care.

Clinical Coordination: Collaborate with admissions nurses, physicians, and the interdisciplinary team to facilitate timely evaluations, referrals, and transitions into hospice care.

Referral Follow-Up: Communicate with referral sources regarding referral status and outstanding clinical documentation, consistent with privacy requirements and organizational policies.

Territory Development: Conduct joint visits with the marketer to healthcare facilities, physician offices, hospitals, and community referral partners.

In-Service Presentations: Provide clinical education and presentations to healthcare professionals regarding hospice criteria, disease progression, symptom management, and end-of-life care.

Community Outreach: Represent Serenity Hospice Care professionally at networking events, facility meetings, educational programs, and other territory-based activities.

Documentation and Reporting: Maintain accurate records of referral interactions, outreach activities, and follow-up efforts in accordance with company procedures.

Compliance: Ensure all referral development activities comply with applicable hospice regulations, patient privacy requirements, and organizational policies.
Required Qualifications

Current, active Registered Nurse (RN) license in the State of New Jersey.

Previous hospice nursing experience is required.

Strong understanding of hospice philosophy, patient eligibility criteria, end-of-life care, and interdisciplinary coordination.

Excellent verbal, written, interpersonal, and presentation skills.

Ability to establish and maintain professional relationships with healthcare providers and facility leadership.

Strong organizational skills and the ability to manage multiple referrals and follow-up activities.

Valid driver's license and reliable transportation for travel throughout Bergen and Hudson Counties.

Ability to work collaboratively with marketing, admissions, and clinical leadership.
