BJC Medical Group is seeking a Board-Certified or Board-Eligible Internal Medicine, Family Medicine, or Geriatric Medicine Physician to join our growing Post Acute Care of Missouri team.

This physician will provide care across multiple BJC-affiliated skilled nursing facilities throughout St. Louis County and St. Charles County , caring for medically complex adult and geriatric patients in a highly collaborative and physician-led environment.

About the Role

The Post-Acute Care Physician serves as an extension of the Hospital Medicine service line, providing comprehensive medical care in skilled nursing and transitional care settings.

Patients frequently require hospital-level complexity management following acute hospitalization, creating an excellent opportunity for physicians who enjoy caring for medically complex populations while maintaining continuity of care.

Key Responsibilities

• Provide admission, follow-up, and ongoing care in skilled nursing facilities

• Manage acute and chronic medical conditions

• Support APPs with complex medical decision-making

• Participate in family meetings and care planning discussions

• Collaborate closely with hospitalists, specialists, and primary care physicians

• Ensure timely and accurate documentation

• Participate in interdisciplinary care conferences

• Support quality initiatives, readmission reduction efforts, and patient outcome measures

• Average census of 12-14 patients per day

• Primarily geriatric patient population

• High-acuity post-acute care patients

• Significant chronic disease management

Ideal Candidate

• Board-Certified or Board-Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine

• Experience in Post-Acute Care, Skilled Nursing, Hospital Medicine, Urgent Care, Primary Care, or Geriatrics preferred

• Excellent communication and relationship-building skills

• Collaborative, team-oriented approach

• Missouri medical license or eligibility for licensure

Role Purpose

The physician will provide professional medical services within the practicing Specialty to the best of physician’s ability through direct patient care and spend additional time as necessary to perform other related duties such as completing medical records, providing MyChart consultations and inbasket management, conducting patient-specific education and collaborating with advanced practice providers and care team members.

Responsibilities

• Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician’s patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician’s hospitalized patients consistent with Medical Staff requirements.

• Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.

• Performs and documents medical histories and physicals in the patient’s medical record as required by hospital medical staff bylaws.

• Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.

• Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician’s patient panel.

• BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
