The Social Worker MSW provides comprehensive psychosocial care and support for patients and families. You would ensure effective care coordination, timely interventions, and smooth transitions across the continuum of care. This role reports to the Director. Essential Job Duties:
Conducts home and facility visits to evaluate patients and families in non-hospital settings Collaborates with Clinical Care Coordinators to identify and manage complex cases requiring psychosocial intervention Performs and documents depression screenings and coordinates appropriate follow-up care Completes follow-up calls or visits within one business day of care encounters Works with Medical Management to interpret and apply beneficiary benefits and services as a liaison Communicates and collaborates with hospital social workers to determine appropriate levels of care and transitions Performs comprehensive psychosocial assessments with early identification of complex issues or changes in condition Incorporates age-specific needs into individualized care plans Manages workload effectively to ensure timely and smooth care transitions Assumes a leadership role in managing catastrophic cases and executing care plans efficiently Facilitates and coordinates transitions for complex patients, including arranging transportation as needed Maintains knowledge of facilities and appropriate community resources Applies federal, state, and organizational regulations related to care planning and continuum of care Communicates care plans with Clinical Care Coordinators, physicians, families, and care facilities Maintains accurate, concise, and timely documentation in medical and departmental systems Participates in departmental, ACO, and community education and in-service programs
Qualifications:
Bachelor’s degree in Social Work required Minimum of five years of clinical experience within the last seven years Strong knowledge of clinical social work practices and Indiana state laws Excellent verbal and written communication skills Experience in case management preferred Ability to manage complex cases and collaborate across interdisciplinary teams Strong organizational, documentation, and time management skills
Licenses / Certifications:
Social Worker certification through the State of Indiana preferred Basic Life Support (BLS) certification required
