Social Worker-MSW

  • Acts as a liaison between the Hospital, the patient, the patient's family, and the various community social and health care agencies.

  • Consults with the Social Services Coordinator regarding activities of the department.

  • Utilizes various methods of communication and assesses the effectiveness of each.

  • Exercises judgement and discretion when consulting with physicians, nurses, patients, patients' families, and significant others, community services, Hospital disciplines and committees, extended care facilities, and other hospitals.

  • Recognizes the importance of patient and employee confidentiality.

  • Reviews medical record and interviews patient and/or family as appropriate, assesses and investigates patient's needs, and acts accordingly.

  • Is knowledgeable regarding pertinent community agencies, institutions, and other resources.

  • Participates in departmental meetings and serves on committees as appointed and requested.

  • Participates in community activities and organizations.

  • Assesses and evaluates patient's needs, including physical, financial, and psychosocial and implements a plan of action to best meet those needs.

  • Demonstrates understanding of the physical and emotional needs of patient and family.

  • Initiates information and referral services to patients and their families who require the assistance of community agencies or other resources.

  • Possesses special aptitudes for understanding and helping people of various backgrounds and cultures.

  • Supervises social work students as requested by the Social Services Coordinator.

  • Contributes to the staff development of social service and other health care personnel.

  • Maintains a quality improvement role by regularly participating in the establishment and monitoring of departmental goals.

  • Assumes responsibility for professional growth and development to meet state mandates for licensure: Seeks assistance in meeting self-learning needs through in-service, continuing education, self-study, and demonstration.

  • Develops social histories when ordered by physician.

  • Shows an understanding of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.

Education, Training & Experience

  • A Bachelor of Science degree in Social Work and licensure as a Social Worker in the State of Indiana required.

  • Two to three years' social work experience would be helpful, particularly with health and crisis social work emphasized.

  • Must have basic knowledge of human behavior.

  • Must have good interviewing and observation skills.

  • Must have good knowledge and understanding of community resources and programs.

  • Must have basic knowledge of medical and social work terminology.

  • Must be able to relate to people and to think and work in an orderly fashion.

  • Must have legible handwriting.

  • Must keep aware of current thinking and trends in social work by reading literature and by attending appropriate meetings.

  • Considerable initiative and judgement required to provide casework services. Supervision provided as needed.

  • Must have very sound oral and written communication skills

  • Must be able to work independently.
