Registered Nurse (RN), Home Health | Full-Time

Serving West Salem, Wooster, Wadsworth

Elara Caring is hiring a full-time Home Health Registered Nurse to serve patients in West Salem, Wooster, and Wadsworth .

Build meaningful patient relationships while using your nursing skills to support recovery, safety, and independence. You’ll provide personalized care, educate families, and partner with a clinical team to help patients manage their health at home.

What You’ll Do

• Complete initial and ongoing assessments to determine patient care needs.

• Provide skilled nursing care according to individualized care plans and physician orders.

• Monitor patient conditions, report changes, and take appropriate action.

• Educate patients and families about their care, health conditions, and safety.

• Coordinate with the care team to support safe, continuous patient care.

• Protect patient and staff privacy in accordance with HIPAA.

What You’ll Need

• Current state license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.

• At least 1 year of nursing experience in a clinical care setting.

• Home health experience preferred, but not required.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
