Home Health RN - Holland, OH

Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) for a Home Health contract in Holland, OH . This is a 13-week assignment providing skilled nursing care and case management to patients in their homes throughout the assigned service area.

Job Details

• Location: Holland, OH

• Pay: $53 to $55.90/hr, mileage reimbursed between patient visits

• Contract: November 2026 – March 2027

• Schedule: Monday–Friday, 8:00 AM–4:30 PM (40 hours/week)

• On-Call/Weekend: One week and one weekend required

• Coverage Area: Toledo, Fostoria, Findlay, and surrounding counties

Responsibilities

• Provide skilled nursing care and case management to patients in their homes

• Complete Start of Care (SOC) assessments and other required home health visits

• Perform comprehensive patient assessments and develop individualized care plans

• Complete OASIS assessments and maintain accurate clinical documentation

• Monitor patient progress and report changes in condition to the appropriate healthcare team

• Coordinate care with physicians, patients, families, and other healthcare professionals

• Travel between patient visits throughout the assigned coverage area

• Participate in required on-call and weekend coverage

Requirements

• Current Registered Nurse (RN) license in Ohio

• Previous home health nursing experience

• Experience completing OASIS assessments required

• Homecare Homebase (HCHB) experience preferred

• Ability to work independently in a field-based environment

• Reliable transportation for travel between patient visits

• Strong communication, organization, and documentation skills

Pay Details: $53.00 to $55.90 per hour
