Pay Range: $25.20 - $39.69 per hour

Rush salaries are determined by many factors including, but not limited to, education, job-related experience and skills, as well as internal equity and industry specific market data. The pay range for each role reflects Rush’s anticipated wage or salary reasonably expected to be offered for the position. Offers may vary depending on the circumstances of each case.

Summary:

The health promotion and disease prevention team functions within the Social Work and Community Health Department and provides all aspects of wellness and health promotion, disease prevention offerings. The Rush Generations membership program is comprised of approximately 18,000 adults who utilize and access Rush for health resources and direct services.

Under the umbrella of Rush Generations programming, adults are provided with health education classes, workshops, health screenings, and other special programs and events. There is a portfolio of evidence-based health workshops and community programs including chronic disease self-management, diabetes self-management, matter of balance fall prevention, and Tomando Control (diabetes management workshops in Spanish). HPDP also offers physical activity, wellness, and mind-body programming to promote health aging and overall well-being. For ongoing support, HPDP provides monthly support groups for caregivers and those managing chronic conditions.

The HPDP program coordinator will assist in all functions and aspects of health promotion programming, working with the program manager and senior manager to fulfill job duties and responsibilities. The Program Coordinator provides program coordination, health education, marketing and member engagement, community outreach, event planning and administrative and data support across Rush Generations and HPDP programming.

The position also supports cross-departmental and Rush-wide initiatives that advance community health, wellness, and engagement. Attendance is required at weekly team meetings and one/one weekly supervision. Job aspects include but are not limited to the following:

Responsibilities:

  • Assist managers with the program planning, coordination, and implementation of HPDP programs, classes, workshops, events, and special initiatives

  • Assist with strategic activities designed to increase the Rush Generation membership base

  • Provides overall programmatic support working in collaboration with HPDP team members

  • Assist with planning and coordination of HPDP events and special programs including Older Americans Month, Annual Volunteer and Partner Celebration, HPDP annual retreat

  • Support Rush Generations Facebook communications and social media content

  • Oversee health education lectures, including lecture planning, speaker recruitment and coordination, preparation of materials and production of virtual programming, as well as several in-person and hybrid health education lectures annually.

  • Coordinate content for the Rush Generations quarterly printed newsletter, monthly electronic newsletter, including gather program updates, class, and event information

  • Support marketing and member engagement efforts for Rush generations, including quarterly material distribution support.

  • Support Rush Generation membership and program recruitment through electronic communication, social media, community outreach, promotional material, and other engagement strategies designed to increase awareness and participation

  • Coordinate and maintain Rush Generation class and program listing across Salesforce and Rush Generations team calendar

  • Provide on-site support for in-person programs, classes and workshops and community events at Rush campus, community partner sites, and Sankofa Village Wellness Center

  • Manage Rush Generations inbox to provide live-answer assistance to emails for RG programming and class registration

  • Becomes trained on Salesforce used for RG membership and classes

  • Provides data input, organization, and evaluation assistance for HPDP program offerings

  • Maintain accurate program records, participant information, registration data, communications and other program documentation

  • Participate in community partnerships and advisory committees, including the Westside Coalition for Seniors Committee and the Village Health and Wellbeing Committee

  • Becomes trained and serves as workshop leader in all evidence-based program offerings

  • Reports directly to senior manager of health promotion programs.
