In this  Field Based HSS **Coordinator ** role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a Field Based position with a Home-Based office.

If you reside within 100 miles from Hidalgo County, TX you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.

**Primary Responsibilities: **

  • Complete face-to-face assessments in members' homes to address unmet needs.

  • Travel up to 75% of the time, including travel up to 100 miles from the assigned work location, to meet business needs.

  • Meet documentation and turnaround times based on state contractual requirements.

  • Time management and organizational skills.

  • Coordinating members' services.

  • Ability to create, edit, save and send documents, spreadsheets and emails

  • Reside within commutable distance of assigned duties

  • Works on Service Plan

Required Qualifications:

  • Must possess one of the following:

  • High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population within a clinical health care role

  • Associate's degree with 3+ years of clinical experience

  • Unrestricted LPN/LVN license in the state of TX with 3+ years of clinical experience

  • CHW certification with 3+ years of clinical experience

  • Intermediate level of proficiency with Microsoft Office, including Word, Excel, and Outlook

  • Access to reliable transportation and the ability to travel up to 75% of the time in this 'assigned area' to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices

  • High speed internet in place of residence

Preferred Qualifications:

  • LVN/LPN License in state of Texas

  • Ability to create, edit, save and send documents, spreadsheets and emails

  • Reside within commutable distance of assigned duties

  • Works on Service Plan

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $24.00 to $43.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
