Full job description
Clinical Liaison (RN/Allied Health Professional)
Location: Philadelphia, PA (On-Site / Embedded at Partner Medical Facility)
Employment Type: Full-Time
Compensation: $100,000-$105,000/annual; with opportunity for quarterly bonus
The Role
Are you an experienced Registered Nurse or Allied Health Professional looking to transition from direct bedside care into a strategic, administrative role focused on clinical collaboration, program outreach, and educating both healthcare providers and patients?
We are seeking a dynamic, mission-driven Clinical Liaison to serve as the primary bridge between our hospital partners and the myLaurel care team. In this fully on-site role, you will be embedded directly inside our partner medical facility. You will serve as the face of our Acute Care at Home program, focusing on relationship-building, patient education, and administrative coordination to drive referrals and ensure seamless patient handoffs.
What You'll Do
As a myLaurel Clinical Liaison, a typical day will include attending daily hospital rounds and huddles, reviewing patient census, completing chart reviews, referral tracking, and providing education and engagement for hospital staff, providers, patients, and caregivers.
• Drive Patient and Partner Engagement: Act as the primary relationship manager on-site. Work alongside hospital case managers, discharge planners, emergency department staff, and physicians to generate referrals and grow our in-home care programs.
• Lead Administrative Coordination: Manage the logistical and administrative workflows of patient transitions, ensuring that the handoff from the hospital or emergency setting to the patient’s living room is flawless.
• Identify Eligible Patients: Use your clinical background to review patient charts, assess eligibility for our acute-care-at-home model, and advocate for safe, alternative care pathways.
• Educate & Advocate: Conduct presentations and daily one-on-one touchpoints with facility staff to educate them on myLaurel’s capabilities, clinical criteria, and patient benefits.
• Solve Problems Strategically: Identify operational barriers to patient referrals in real-time and collaborate with the myLaurel leadership team to implement.
What You Bring
• Active Registered Nurse or Allied Health Professional License: Current, unencumbered license in the State of Pennsylvania
• Experience: 2+ years of experience in an acute care clinical setting. Previous experience or a strong interest in healthcare business development, clinical sales, discharge planning, or utilization review is highly preferred.
• Relationship Builder: Excellent communication and interpersonal skills, with the natural ability to establish trust, integrate into a facility's culture, and build rapport with busy medical professionals.
• Administrative Acumen: Strong organizational skills and a desire to manage program logistics and care transitions rather than providing direct, hands-on patient care.
• On-Site Presence: A highly engaged professional who thrives in a bustling medical facility environment and is comfortable being the primary, in-person representative for myLaurel.
• Tech-Savvy: Comfortable navigating various EMRs (Electronic Medical Records) and utilizing CRM software to track referral metrics and patient engagement.
Why Join myLaurel?
- Career Growth: The perfect opportunity to pivot your clinical expertise into the business, administrative, and growth side of healthcare.
• Comprehensive Benefits: Medical, Dental, and Vision insurance.
• Work-Life Balance: Generous Discretionary Time Off (DTO) and paid holidays.
• Financial Wellness: 401(k) retirement plan.
• Impact: The opportunity to be on the ground floor of a transformative healthcare model that keeps patients out of the hospital and in their homes.
About myLaurel
myLaurel was founded on the belief that a meaningful portion of hospital care can—and should—be delivered in the comfort of home. As a tech-enabled medical group, we provide on-demand acute and transitional care to frail, elderly, or medically complex patients at home—helping them avoid the traditional pathway of ambulance, emergency department, hospital admission, and post-acute facility care.

We stand behind our outcomes by putting our fees at risk, demonstrating both accountability and confidence in the results we deliver.
