City/State Charlottesville, VA Work Shift First (Days) Overview: Sentara Home Care Services in Charlottesville is hiring a Licensed Practical Nurse.

Position Status: Full Time - Day Shift

Position Location: Charlottesville, VA. This position is 100% travel in personal vehicle. Mileage reimbursement will be paid for miles driven between patient visits.

Overview The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

Education

• Trade School Graduate (Required)

Certification/Licensure

• LPN License (Required) - Active in Virginia
• BLS (Required within 90 days)

Experience

• 2 years LPN experience minimum (required)
• Serve as patient advocate in the prevention of illness or disease
• Provide care and treatment for patients and clinical support and teaching for patient and family members
• Customer service, testing, medication administration and the implementation of nursing interventions and care plans
• Support a holistic approach to patient care

The base pay range for full-time employment is $28.57 - $43.04 hourly.
