Schedule: Full time Shift: Day (United States of America) Address: 120 N OAK ST City: HINSDALE State: Illinois Postal Code: 60521 Job Description:
Sign On Bonus For eligible applicants, up to $10,000
Schedule: Full-Time/Day Shift: Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation. Location: Barington, Schaumburg, Batavia, Carol Stream, Hoffman Estates, Itasca, Medinah, Roselle, St Charles, and Steamwood Area with travel to LaGrange Hospital for meetings/Orientation etc.
- Implements and monitors patient care plans to ensure effective treatment.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as the primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders accurately and promptly.
- Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status.
- Administers medications and solutions as required.
- Documents all phases of patient status during care.
- Participates in the transfer of patient information to ensure continued quality care.
- Leads and mentors staff to ensure optimal performance and competency in care delivery.
- Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations.
- Develops and maintains positive working relationships within the practice and other departments.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
Field of Study:

Work Experience:
- 1+ acute care clinical experience [Preferred]
- 1+ home health experience [Preferred]

- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:
