Hours: Monday - Friday 8a-5p MOUNTAIN TIME

Pay Range: $18-$19/hour

Type: 2-month contract - possibility of extension

Patient Outreach Scheduler (Remote) - Job Summary

The Patient Outreach Scheduler is responsible for conducting high-volume outbound calls to patients across Colorado, Utah, and Idaho to support preventative care and patient engagement initiatives. This role focuses on scheduling Annual Wellness Visits, care-gap appointments, and other targeted healthcare services while providing an excellent patient experience. The scheduler utilizes Epic, APM, and other scheduling systems to coordinate appointments, document outreach efforts, and maintain accurate tracking records in a remote work environment.

Key Responsibilities

  • Conduct outbound patient outreach using approved scripts.

  • Schedule Annual Wellness Visits and other designated healthcare appointments.

  • Review patient information and provider availability in Epic, APM, and related systems.

  • Document outreach attempts, appointment outcomes, and follow-up activities accurately.

  • Maintain patient outreach lists, scheduling trackers, and work queues.

  • Follow established workflows, privacy guidelines, and scheduling protocols.

  • Escalate clinical questions and complex scheduling concerns to the appropriate teams.

  • Collaborate with internal teams via email, Teams, and other communication platforms.

  • Meet productivity, quality, attendance, and patient satisfaction goals while working independently.

Ideal Candidate Profile

  • HS Diploma or GED

  • Previous medical office scheduling experience - minimum 6 months

  • EHR/Scheduling software experience - EPIC strong preferred

  • 6+ months of relevant experience in healthcare scheduling, patient outreach, call-center operations, customer service, medical-office support, or a similar telephone-based role

  • 6+ months of customer/patient communication experience, including professional telephone communication

  • Demonstrated ability to accurately schedule appointments and follow established scheduling procedures

  • Ability to manage patient lists, work queues, tracking files, and follow-up activities

  • Strong verbal communication and active-listening skills

  • Strong attention to detail and accurate documentation skills

  • Basic proficiency with email, Microsoft Teams/chat, Microsoft Office, and electronic documentation

Pay Details: $18.00 to $19.00 per hour
