Mercy Care Atlanta is seeking a Nurse Practitioner (NP) to join our:

Street Medicine Team

Mercy Care's Street Medicine Team brings healthcare directly to individuals experiencing homelessness and others who face significant barriers to accessing traditional healthcare services. Rather than waiting for patients to come to a clinic, our providers meet people where they are, including shelters, encampments, transitional housing sites, and community locations throughout metro Atlanta.

As a Street Medicine Nurse Practitioner, you will build trusting relationships with some of the community's most vulnerable populations while providing compassionate, trauma-informed care in nontraditional settings. This role combines clinical practice, outreach, advocacy, and care coordination to address immediate health concerns while helping patients navigate a path toward long-term stability and ongoing healthcare services.

What You'll Do as a Street Medicine NP

  • Deliver primary and preventive healthcare services in community-based and outdoor settings.
  • Assess, diagnose, and treat acute and chronic medical conditions among individuals experiencing homelessness.
  • Perform health screenings, wound care, medication management, and chronic disease monitoring.
  • Collaborate with social workers, community health workers, behavioral health professionals, and community partners to address social determinants of health.
  • Connect patients to housing resources, behavioral health services, specialty care, substance use treatment, and other supportive services.
  • Develop individualized care plans designed to improve health outcomes and reduce barriers to care.
  • Document patient encounters and coordinate follow-up care across Mercy Care clinics and community partners.
  • Participate in outreach activities focused on engaging hard-to-reach populations and establishing continuity of care.
  • Advocate for patients and serve as a trusted healthcare partner for individuals who may have had limited access to healthcare services.

Minimum Qualifications

  • Graduation from an NLN-approved MSN Nurse Practitioner program or a certified Nurse Practitioner program with three years of NP experience.
  • Georgia RN license required.
  • Two years of RN experience preferred.
  • Experience working with diverse and underserved populations preferred.
  • Ability to diagnose and treat patients in health and illness using health promotion, disease prevention, and therapeutic interventions.
  • Knowledge of health problems, issues, and needs of the specific target populations.
