Advanced Practice Provider-Outpatient Transplant (Full-time)

Piedmont Healthcare is seeking a Nurse Practitioner or Physician Assistant to join our Transplant Outpatient Team. This role is responsible for the ongoing management and coordination of care for liver transplant patients in a collaborative, multidisciplinary setting.

The ideal candidate is clinically strong, highly organized, and comfortable practicing with a high degree of autonomy while managing complex transplant patients.

• Provide comprehensive evaluation and management of liver transplant recipients.

• Develop and implement individualized treatment plans based on clinical assessment, laboratory, and diagnostic findings.

• Monitor graft function, immunosuppressive therapies, and post-transplant complications.

• Educate patients and families regarding transplant care, medications, and lifestyle modifications.

• Coordinate care with transplant surgeons, hepatologists, coordinators, pharmacists, and other members of the multidisciplinary team.

• Perform minor procedures within scope of practice.

• Obtain informed consents as appropriate.

• Participate in transplant program quality initiatives, research activities, and multidisciplinary conferences.

• Serve as a clinical resource for staff and support educational initiatives.

• Participate in after-hours call coverage approximately once per month, including holidays.

Position Details:

Outpatient, non-OR role

Full-time: 40 hours a week (Monday-Friday), 10 hour shifts 4 days a week

Qualifications

Qualifications:

New Grads accepted.

Minimum Education Required:

PA: Graduate of a Physician Assistants program with current license for the Georgia State Composite Board

OR

NP: Graduate from a school of nursing with current Georgia license for Advanced Practice Nursing and a Master’s degree in Nursing
