Overview As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team. Requirements
- Graduate of an accredited Licensed Practical Nurse educational program
- Current Licensed Practical Nurse / LPN license within the state of practice
- Current CPR certification
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Strong initiative, follow through, process orientation, attention-to-detail
- Reliable transportation, valid driver’s license and current automobile insurance
