Our client is seeking a dedicated and proactive Registered Nurse - Assisted Living Facility Nursing Case Manager to join this supportive team.

Responsibilities:

• Perform thorough assessments to understand each resident's unique needs.

• Develop, implement, and evaluate individualized care plans.

• Administer medications and treatments with precision and care.

• Educate residents and their families on health management and wellness.

• Collaborate with our interdisciplinary team to ensure optimal resident outcomes.

• Be a trusted advocate for our residents' health and comfort.

Flexible Schedule for Your Work-Life Balance:

• Work 4 days a week! Enjoy an extra day off each week to recharge and pursue your passions.

• Flexible schedule with weekend rotation.

• Consistent daytime hours: 7:00 a.m. to 5:30 p.m.

Qualifications:

• Compassionate, empathetic, and patient-centered.

• An excellent communicator, capable of building rapport with residents, families, and staff.

• Proactive in identifying resident needs and developing solutions.

• Skilled in clinical assessment and care plan development.

• Holds an active Registered Nurse (RN) license in New York.

• Must have BSN
