Richmond Community Services is looking for a dynamic Manager of Community Nursing Services to lead our community nursing team and strengthen clinical coordination across our residential programs.
This is an opportunity to make an impact beyond one nursing station. You'll help lead a team of community nurses, provide clinical guidance, coordinate responses to emerging health concerns, and ensure the people we support receive timely, proactive, and compassionate care.
What You'll Do
-
Lead, mentor, and support community nursing staff across multiple residential locations.
-
Provide clinical guidance and triage coordination for emerging medical concerns, helping determine appropriate next steps and escalation.
-
Apply an understanding of PONS (Plan of Nursing Services) to support individualized nursing oversight, coordination, and continuity of care.
-
Oversee nursing assessments, medication reviews, clinical notes, healthcare appointments, and follow-up documentation.
-
Coordinate communication among nurses, physicians, healthcare providers, Residential Managers, and Lead DSPs.
-
Support residential teams in responding to changes in health status and addressing clinical concerns proactively.
-
Promote consistent nursing practices, quality documentation, and compliance with OPWDD requirements and Agency policies.
-
Identify opportunities to improve clinical workflows, communication, and the delivery of community nursing services.
-
Participate in the nursing on-call rotation approximately once every four weeks, providing clinical guidance and support when needed.
What We're Looking For
-
Current New York State Registered Nurse (RN) license in good standing.
-
Nursing leadership, supervisory, or clinical coordination experience.
-
Understanding of PONS and nursing oversight within OPWDD-regulated residential services.
-
Strong clinical assessment, triage, and care coordination skills.
-
Experience supporting individuals with complex healthcare needs preferred.
-
Knowledge of medication administration, nursing documentation, and regulatory requirements.
-
Excellent communication, critical-thinking, and organizational skills.
-
A collaborative leader who can develop nursing staff while building strong partnerships with residential operations.
Why Join Richmond?
We recognize that strong nursing leadership requires both professional support and a sustainable work environment.
What we offer:
-
Sign-On Bonus: Additional incentive for joining our nursing leadership team.
-
Hybrid Flexibility: One remote workday per week.
-
Predictable On-Call Rotation: Approximately one week every four weeks.
