Full job description

The SLS/CES-Children's Case Manager-Float is responsible for covering a variety of duties, which could include responding to contacts seeking information regarding services and supports for members, organizing information for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to IDD and Non-IDD members and their families as appropriate. The SLS/CES Case Manager-Float will provide Case Management support for members when a primary case manager is not available due to position vacancies, employee leave or during times of high member enrollment. Spanish bilingual preferred with a $1.00 increase in pay rate. Requirements: - Bachelor's degree required; or five years of relevant experience in the field of LTSS, which includes some combination of education and relevant experience appropriate to the requirements of the position. - Knowledge of cognitive disabilities: cause, treatment, services for support, education, employment, recreation, therapy, and related medical terms/medications. - Driving is a requirement for this position. Must be able to drive company vehicles.
