Responsibilities

• Deliver skilled nursing care in a home setting

• Administer medications, treatments, and physician-ordered interventions

• Monitor patient conditions and report changes to the care team

• Implement individualized care plans

• Complete accurate documentation and charting

• Collaborate with physicians, caregivers, and interdisciplinary healthcare teams

Required:

• Active Licensed Practical Nurse (LPN) license

• Current BLS/CPR certification

Preferred:

• 1+ year of nursing experience

• Experience with tracheostomy (trach), ventilator (vent), G-tube, enteral feeding, seizure management, or other skilled pediatric/homecare cases preferred but training available
