Lead Healthcare Transformation for Older Adults

At CenterLight Healthcare Systems, we believe healthcare should be coordinated, compassionate, and centered around the whole person.

As a Clinical Service Director (CSD), you will provide leadership for interdisciplinary healthcare teams while helping older adults remain safely and independently in their communities.

WHAT YOU’LL DO

Clinical & Operational Leadership

  • Provide leadership and oversight for Interdisciplinary Team (IDT) operations and participant outcomes.

  • Direct and support clinical and operational managers across multiple service lines.

  • Ensure development and evaluation of participant-centered care plans.

  • Monitor operational, quality, and clinical performance metrics.

  • Collaborate with organizational leaders to enhance care delivery processes.

Quality & Regulatory Excellence

  • Ensure clinical documentation is accurate and compliant.

  • Lead quality improvement initiatives and support regulatory compliance readiness.

  • Oversee incident investigations and participant concerns.

  • Conduct audits and reviews to promote compliance.

Team Leadership & Development

Mentor, coach, and develop interdisciplinary leaders and staff.

Foster a culture of accountability, collaboration, innovation, and excellence.

WHAT WE’RE LOOKING FOR

Required

  • Active and unrestricted RN license in New York State.

  • Bachelor of Science in Nursing (BSN).

  • Minimum 3 years recent acute care nursing experience.

  • Experience working with elderly populations in PACE, MLTC, CHHA, or similar settings.

  • Demonstrated leadership experience.

Preferred

  • 2-3 years supervisory or management experience.

  • PACE experience.

  • Bilingual skills.

  • Experience leading interdisciplinary healthcare teams.

Salary Range (Min-Max):$135,000.00 - $145,000.00

CenterLight Healthcare PACE, a Program of A (https://centerlighthealthcare.org/care-services/) ll-InclusiveCare for the (https://centerlighthealthcare.org/care-services/) Elderly, serves adults 55+ throughout New York City, Westchester, Nassau, and Western Suffolk Counties. Designed for those with Medicare and/or Medicaid, the program empowers older adults to continue to live independently at home and in their communities. CenterLight’s PACE program is one of the largest nonprofit PACE programs in the country and operates 11 PACE centers (https://centerlighthealthcare.org/about/where-we-are/) and one Alternative Care Setting (ACS) where participants can receive on-site socialization, social and personal care services, therapeutic recreation (https://centerlighthealthcare.org/what-is-therapeutic-recreation/) , meals, and transportation. Our PACE centers also offer medical services, rehabilitation, prescription and over-the-counter (OTC) medications to thousands of New Yorkers by an interdisciplinary team (IDT) of doctors, nurses, physical therapists, nutritionists, recreational therapists, and other professionals who work together to provide care as unique as the diverse participants enrolled in the plan.
