LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV-related issues, and a passion for advocacy.

Responsibilities:

  • Support individuals living with HIV and connect them to care.

  • Conduct outreach and educational initiatives.

  • Build relationships with community organizations.

  • Advocate for patients and address barriers to care.

  • Provide HIV-related education and resources.

  • Collaborate with healthcare teams to support patients.

  • Maintain records and complete assigned tasks.

Qualifications:

  • Experience in community outreach or healthcare.

  • Knowledge of HIV prevention and support services.

  • Strong interpersonal and organizational skills.

  • Bilingual in Hai tian Creole (preferred).
