NW Works Mission: Our mission is to empower adults with disabilities and individuals with barriers to employment to build skills and secure and sustain meaningful employment. Job Summary: This position is responsible for the creation of Person-Centered Plans for individuals with disabilities across their authorized programs. In addition to planning (case coordination) this role will be responsible for scheduling clients across programs and supporting the staffing of the programs as designated by their supervisor. Essential Functions: Leads each DSP on his/her specified caseload to ensure that PCPs (Person Centered Plans) are structured for each client. Ensures that opportunities offered to clients respect the principles of dignity of risk, and person centeredness with a primary focus on community inclusive opportunities. Leads
