Schedule: Full time Shift: Day (United States of America) Address: 2525 S DOWNING ST City: DENVER State: Colorado Postal Code: 80210 Job Description:

  • Promotes knowledge of oncology and its effects for patients, support partners, and care providers.
  • Meets with patients and support partners after diagnosis to provide counseling and referral services specific to their needs.
  • Collaborates with the multidisciplinary team to provide ongoing support for patients and oversee the plan of care.
  • Educates and collaborates with the multidisciplinary team to plan and participate in quality initiatives and research projects.
  • Reviews patient information and assess patients for emotional needs and risk factors.
  • Serves as a customer service advocate for patients and support partners.
  • Serves as the primary point of contact for providers and patients requiring the Oncology Licensed Social Worker (OLSW).
  • Works with Navigators to provide Survivorship Care Plans to patients.
  • Performs patient and family evaluations and histories, providing psychosocial support through individual or family counseling as needed.
  • Acts as a resource to Infusion/Oncology Practices for patient needs and collaborates with patients to identify potential barriers to achieving oncology treatments and set health goals for quality of life.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • Able to communicate effectively in English, both verbally and in writing [Required]
  • Additional languages [Preferred]

Education:

  • Master's [Required]

Field of Study:

  • in Licensed Social Work

Work Experience:

  • 3+ of recent social work experience in oncology [Preferred]

Pay Range:
