Job Description:

Schedule: Full-Time Salaried Position

Hours: Mon-Friday 8am-5pm.

Location: Sits in our Inpatient Unit with Hospice SFM and could round to Palm Coast and Palm Coast Parkway as needed for face to face visit and within the community.

• Evaluates the health status of selected patients, orders treatment plans and provides education to patients and their families under the supervision of a licensed physician.

• Performs clinical evaluations and reviews diagnostic studies of patients under the supervision of a licensed physician.

• Participates in interdisciplinary team conferences of the facility teams, as needed. Interacts with patients and families to manage the psychological, social and family issues regarding hospice and the services rendered.

• Collaborates and consults with the hospice management team to facilitate problem solving toward meeting patient needs.

• Completes all documentation timely and accurately in the patient records on daily visits.

• Participates in staff meetings, department meetings, in services and other related activities. Attends educational and developmental seminars and meetings, as deemed appropriate.

• Provides necessary billing documentation in a timely manner to the hospice Finance department.

• Performs all other duties as assigned

Education:

• Master's of Nursing [Required]

Field of Study:

• N/A

Work Experience:

• 3+ years of experience in medical nursing, hospice and/or critical care [Required]

• 2+ years of experience as an APRN [Required]

Licenses and Certifications:

• Advanced Practice Registered Nurse (APRN) [Required]

• Basic Life Support - CPR Cert (BLS) [Required]

• Driver's License (DL) [Required]

Pay Range:

$44.34 - $82.47
