Schedule: Full time Shift: Day (United States of America) Address: 11801 SOUTH FWY City: BURLESON State: Texas Postal Code: 76028 Job Description:
Educates patients about available health care benefits and community resources. Makes necessary referrals to appropriate health care services and community resources. Interacts frequently with patients, provider offices, and members of the interdisciplinary team. Demonstrates outstanding customer service skills, including empathy, active listening, and helpfulness. Communicates effectively both in writing and verbally with patients, team members, and external partners. Documents patient interactions and outcomes accurately in the electronic medical record. Researches and distributes department-approved community resources to patients. Escalates issues and barriers promptly to ensure timely resolution. Collaborates with the interdisciplinary team to ensure coordinated and efficient patient care. Maintains respectful and supportive relationships with patients, team members, and external partners. Supports operational goals by maintaining expected attendance and achieving productivity targets. Performs other duties as assigned.Knowledge, Skills, and Abilities:

- Knowledge of Microsoft Office (Word, Excel, PowerPoint, and SharePoint) and Microsoft 365 Software [Required]
Education:
- Bachelor's in Social Work, Public Health, Psychology, or Community Health Worker [Preferred]
- High School Grad or Equiv [Required]
Work Experience:
- 2+ years of experience working remotely in a patient-facing, healthcare-related role or as a Community Health Worker or Patient Navigator or Transitions of Care related role [Preferred]

Pay Range:
