Description

This position supports grant performance objectives through documentation, data collection, patient tracking, and reporting activities that demonstrate the impact of grant-funded services and interventions. They will work closely with Mary Greeley Medical Center’s Program Coordinator for the Combat Cancer Grant.

Responsible for supporting patients throughout the cancer care continuum by reducing barriers to care, facilitating access to services, promoting adherence to recommended screenings and treatments, and connecting patients with appropriate clinical and community resources in accordance with McFarland Clinic’s Core Values and Promise.

Employed in a grant-funded role for the duration of a five-year program.

EXAMPLES OF DUTIES: (This list may not include all of the duties assigned.)

  • Serve as a primary point of contact for the Hub (Mary Greeley Medical Center) participating in Combat Cancer Grant-funded programs and services.

  • Enter and track required patient and program data within designated databases, electronic health records, and grant reporting systems.

  • Monitor patient progress and maintain tracking tools to ensure appropriate follow-up and continuity of care.

  • Collect and document grant-required performance measures and outcome data.

  • Assist with monthly, quarterly, and annual reporting activities required by the Combat Cancer Grant.

  • Support data validation efforts to ensure completeness, accuracy, and compliance with grant reporting requirements.

  • Participate in quality improvement initiatives aimed at improving patient access, engagement, and outcomes.

  • Assist in compiling patient success stories, outcomes, and other information demonstrating grant impact.

  • Coordinate care across healthcare providers, clinics, hospitals, community organizations, and support services.

  • Assist patients in navigating screening, diagnostic, treatment, survivorship, and follow-up care services.

  • Facilitate referrals to appropriate clinical services and community resources.

  • Support timely scheduling of appointments, screenings, treatments, testing, and follow-up visits.

  • Identify and help implement efforts to improve access to cancer prevention, screening, treatment, and survivorship services throughout the service region.

  • Conduct outreach to patients who are overdue for screenings, follow-up appointments, or recommended treatments.

  • Collaborate with MGMC and spoke partners, including Dallas County Hospital, McFarland Clinic-Marshalltown, Iowa Specialty Hospital, and other participating organizations, to ensure coordinated patient support services across the region.

  • Participate in multidisciplinary team meetings, care conferences, and grant-related workgroups.

  • Foster positive working relationships with healthcare providers, community organizations, public health agencies, and social service partners.

  • Assist with community outreach, education, and cancer awareness initiatives supported by the grant.

  • Provide education regarding available services, treatment resources, financial assistance programs, transportation options, and support services.

  • Assist patients and caregivers with understanding care plans and accessing available community resources.

  • Support transitions of care between providers, settings, and levels of care.

  • Maintain accurate and timely documentation of patient encounters, interventions, referrals, and outcomes.

Education

  • Associate degree in Nursing, Social Work, Human Services, Healthcare Administration, Public Health, or related field required.

Days/Hours: Part Time: 20-30 hours/week

Experience

  • Experience working with underserved or vulnerable populations preferred.

  • Experience in cancer care, patient navigation, or grant-funded programs preferred.

Pre-employment drug screen and criminal history background checks are a condition of hire.
