Devereux Advanced Behavioral Health Connecticut provides programs and services for children and adults with IDD. Located throughout the Washington/Waterbury, CT area, our IDD Program provides high quality treatment for individuals with IDD. We work with our individuals on campus and CLA spaces to provide the highest quality services and meet our individual's needs.

Loacted in Washington/Waterbury, CT the Care Coordinator will work with clinical, operations, and administrative staff on the overall coordination of individual care. Responsibilites could include: Utilization Management, Referral and Admission Processes, Medical Records, Educational Coordination, and manitanance of documantaion adccordingly. This position will manage unexpected events, and acts on opportunities to improve the services that we provide to our customers.

Schedule: Monday through Friday 9am to 5pm

Qualifications

Our Ideal Candidate:

  • High School Diploma or GED required with four (4) years of related experience OR a Bachelor's Degree in Human Services or related field with One (1) year of related experience.

  • Knowledge and background in assessment and treatment planning development.

  • Excellent communication skills with ability to interact with numerous staff and clients successfully.

  • Ability to make decisions and meet deadlines.
