Employment Type: Full timeShift: Day Shift Description: St Mary's Hospital a member of Trinity Health Of New England is looking for an Extended Care Facility (ECF) Transfer Coordinator to join our Case Management team.

The ECF transfer Coordinator organizes all aspects of a patient’s referral and transfer to an extended care facility, including the maintenance of tracking and documentation and the collaboration with other members of the multidisciplinary team

You Will Be Responsible For:

  • Maintains open communication with the patient, family, Nurse Care Manager, medical staff, insurance company and all others directly involved with the referral of the patient.
  • Collaborates and communicates with staff, patients and families to facilitate effective transfers to appropriate facilities.
  • Provides answers to all clinical questions on referrals made by ECF admission coordinator and ECF nursing staff.
  • Maintains proficiency with office computer systems. Performs administrative duties in a fast-paced environment
  • Promotes collaborative relationships with ECF’s and community agencies through open, honest and professional communication and problem solving.

Requirements:

  • High School/GED Required, Associates Degree Preferred
  • Previous Healthcare experience Preferred
  • Knowledge of medical terminology Preferred
  • Strong organizational, verbal, and writing skills required.
  • Experience with Excel, PowerPoint, Word required
