Description

The Registered Nurse fulfills the nursing function identified in Legacy's approved SBIRT staffing plan and supports integrated SBIRT and behavioral health services through health screening, nursing assessment, health education, chronic disease support, medication education and monitoring, care coordination, and collaboration with primary care and behavioral health providers. The Registered Nurse identifies physical health needs and risk factors within nursing scope, provides age- and developmentally responsive health education, supports medication reconciliation and adherence activities when assigned, and coordinates follow-up with primary care, specialty medical, behavioral health, and substance use providers. The Registered Nurse collaborates with Co-Occurring Counselors, the Care Navigator, Peer Recovery Specialist, prescribers, and community partners to support whole-person care and continuity following SBIRT screening and referral. Preventive or infectious-disease screening, vaccination coordination, and other health activities may be performed when included in approved project workflows and consistent with nursing scope, Agency protocols, and applicable orders. The primary assignment during time charged to the SBIRT award is nursing and integrated health support for the SBIRT project. Outpatient nursing duties may be performed during SBIRT-funded time when they directly support SBIRT participants, health screening, referral completion, project integration, or sustainability.

Legacy’s Screening, Brief Intervention, and Referral to Treatment (SBIRT) Program is funded through a grant from the Substance Abuse and Mental Health Services Administration (SAMHSA). The program provides early identification and intervention for substance use concerns for individuals age 12 and older, with a particular focus on youth and young adults ages 12–21. The program operates across Atlantic, Burlington, and Camden Counties and integrates SBIRT into outpatient behavioral health, healthcare, school, mobile outreach, and community-based settings.

Services include evidence-based substance use screening, brief intervention and brief treatment when indicated, and referral to treatment for individuals needing more intensive substance use services. The program also provides care navigation, warm handoffs, follow-up, peer and recovery support, and coordination with behavioral health, primary care, substance use treatment, and community resources to reduce barriers and improve continuity of care.

Hours

Position is full time and includes the following:

Qualifications

Education/Experience:

Current, unrestricted New Jersey Registered Professional Nurse license required. Minimum three years of behavioral health, psychiatric, integrated care, or community health nursing experience preferred. Experience serving adolescents, young adults, and adults with substance use disorders, co-occurring mental health disorders, and chronic or complex health conditions preferred. Experience collaborating with primary care providers and multidisciplinary teams highly desirable.
