Job Summary and Responsibilities

As a Registered Nurse (RN), you will be a pivotal healthcare professional, delivering compassionate, high-quality care that truly impacts our patients' well-being and recovery.

  • Facilitates coordination of a multidisciplinary plan of care to guide patient progress to optimal care transitions

  • Performs evaluation of patient’s care and progress for the episode of care and/or across the continuum of care for the identified population

  • Reviews patient medical records to determine justification of admission, treatment and continued stay criteria

  • Determines and facilitates the appropriate post-hospital discharge destination for a patient

  • Identifies patient needs, capacity and preferences to determine what is required for a smooth and safe transition to patient’s discharge destination

  • Actively involves patient, or individual acting on patient’s behalf, in planning for discharge

Job Requirements

Required

  • Diploma Of Nursing Graduate of accredited professional nursing education program

  • Registered Nurse: MN

Preferred

  • Bachelors Degree in a healthcare field

  • Experience as a Discharge Coordination and/or in Case Management
