Registered Nurse 2 (RN2) - Discharge Planning Non-Permanent - Internal Only Washington State Penitentiary (WSP) Walla Walla, WA Monthly Salary Range: $8,151 to $13,362

Current Available Shift:

  • Monday - Friday 0800 - 1630

WA State Penitentiary is seeking an Internal applicant for the RN2-Discharge Planning position. This position is a part of a multidisciplinary team reporting to a RN3 that supports the Department’s mission to improve public safety by support transition and reentry through development of a Continuity of Care Plan for patients with significant physical and/or mental disorders who will soon release to the community. Continuity of Care Plans are extended to patients, assessed by Health Services in each facility, to be at highest risk for immediate serious medical problems upon release. The Continuity of Care plan will include pre-release planning, referrals, and coordination of services with DOC Health Services Staff, Counselor staff, Security staff, and Administration staff. Community reentry coordination includes planning with patients, families, Community Corrections, and community resources for medical benefits, financial assistance, special housing, or facility/institution placement. Continuity of Care strives to promote/maintain the medical/mental health stability achieved while incarcerated and encourages community support to the released patients, with the goals of successful community reentry and reduction of recidivism. This position, in collaboration with the facility Health Services reentry team, focuses on the Department’s Strategic Goal of Continuity of Care Planning for Health Services patients. Initiate and support Continuity of Care planning and coordination of information and resources for patients referred to case load. Foster positive professional relationships with Community Partners in reentry efforts to strengthen teamwork related to transition planning, reach-out initiatives. Maintain current, complete and confidential patient records in accordance with Health Service Policies and Procedures.

  • Provide central coordination, in collaboration with other Health Services reentry staff, of the patient transition caseload and facilitate communication among the pertinent healthcare team members and institutional staff as appropriate.
  • Elicit participation of the team in developing, assessing, and revising individual patient Continuity of Care plans. This may include contacting institution Health Services staff, Program Services staff, Security staff, Transport staff, Administration staff, and when appropriate, the patient family.
  • Initiate and coordinate Community referrals to appropriate service providers and agencies such as Disability Services, Social Security, Dialysis services, VA services, Community Care Facilities, Personal Care providers, and Community Corrections transition services staff.
  • Initiate and coordinate Medical Facility placement for specialized care after release. Work with Community resources and providers to facilitate eligibility and placement process and to locate appropriate care facilities. Work with Care Facility staff to ensure appropriate continuity of medical care. (Plan with Community Corrections and DOC Administration to assure any necessary County waivers or possible adjustments in parole plans due to medical needs.)
  • Assess durable medical equipment needs and provide equipment necessary for immediate support upon release, such as wheelchairs, walkers, canes, or glucometers.
  • Provide follow-up or post-release problem solving with Community resources and services as appropriate to facilitate the transition period.
  • Provide resources, contact, and process information to facilitate communication between DOC and Community team.
  • Provide assistance to obtain necessary medical transition information necessary to continuity of care
  • Accept opportunities to share DOC transition efforts to improve re-entry success-through improved communications and meeting with community team members.
  • Support and facilitate accessibility of medical and financial benefits available in the community through pre-release application processes for these benefits.
  • Identify community resources through community contacts for Nursing Homes, Secure facilities, Hospice facilities, and Adult Foster Care facilities.
  • Coordinate EMP by receiving referrals for potential candidates, facilitating screening with the Facility Medical Director, providing necessary information and support through the investigation process and assisting, as part of the facility reentry team, with Continuity of Care planning upon identification of approval.

Coordinate the Re-Entry Navigation to ensure MOUD services are part of Continuity of Care planning.

  • Work with MOUD transition staff to identify community resources for continuation or initiation of MOUD services at release.
  • Educate potential candidates about the MOUD program expectation and Narcan use upon release to the community.
  • Facilitate lab tests, medical appointments, follow up services needed and release medications upon release.

Model pro-social behaviors; reinforce positive behaviors; redirect negative behaviors; and effectively communicate observed behaviors to responsible program staff, administrators, and supervisors. Initiate the Continuity of Care planning process by notification to all DOC Facility Health Services of patients with identified medical needs scheduled to be released within 6 months.

  • Accept referrals of higher needs patients. Collaborate with the facility Health Services reentry team to identify high needs patients.
  • Conduct patient interviews to determine specific medical, mental health, substance use disorder and other transition needs. A personalized Continuity of Care plan is developed based upon patient and staff interviews, medical record reviews, and review of transition staff release planning.

Other

  • Emergency assignment to help with security in the event of a patient disturbance.
  • Other duties as assigned for Health Services.

Registered Nurse 2 (RN2)

  • Initial and continuous unencumbered licensure in the State of Washington as a Registered Nurse OR An unencumbered Multistate License from a participating Compact Licensure State and completion of the MLS demographic survey.

  • One year experience working as a Registered Nurse.

  • Must be American Heart Association Basic Life Support (BLS) (Advanced Cardiac Life Support or other will not substitute for BLS)

  • Proficient using computer-based programs to schedule, monitor and track patient information, perform data entry and word processing. Communicate via e-mail as well as other programs that may be needed to complete assigned work.

  • Ability to work independently and with limited supervision.

NOTE: Must have initial and continuous unencumbered/unrestricted licensure as a Registered Nurse.
