Job Description:

• Facilitates timely scheduling with coordinators, supervisors, physicians, and patients.

• Monitors high-risk cardiovascular patients and provides necessary support.

• Participates in performance improvement activities to enhance cardiovascular service delivery.

• Maintains professional relationships with all customers and stakeholders.

• Acts as a liaison to physicians’ offices regarding proper orders, medical records, consents, and other required documentation.

• Coordinates multidisciplinary cardiovascular services including cardiology, cardiothoracic surgery, and vascular surgery.

• Develops and implements evidence-based measures to improve the quality of cardiovascular care.

• Ensures compliance with all regulatory guidelines related to cardiovascular services.

• Supports and assists with data collection for process and quality improvement initiatives.

• Manages patient access and scheduling within the health system for cardiovascular services.

• Provides patient education and serves as a resource for patients and families during diagnosis and treatment.

• Communicates patient status and needs to healthcare teams and collaborates with other providers to sequence care.

• Other duties as assigned.

• Education:

• Associate's of Nursing [Required]

• Bachelor's of Nursing [Required]

• Master's [Preferred]

• Work Experience:

• 2+ years acute care experience [Preferred]

• 2+ years in cardiac care [Preferred]

• Licenses and Certifications:

• Registered Nurse (RN) [Required]

• Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:

$33.08 - $58.04
