Oncology Companion 100% Remote | Anywhere in the Contiguous U.S. | Oncology Care Model Are you passionate about supporting cancer patients and their families through some of life's most challenging moments? Join our innovative Oncology Care Model team as an Oncology Companion, serving as a trusted guide, advocate, and single point of contact throughout a member's cancer journey. About the Role The Oncology Companion provides a concierge-style experience for members receiving oncology care. Acting as the primary point of contact, you'll build meaningful relationships with members and caregivers while helping them navigate treatment, benefits, healthcare resources, and barriers to care. Working closely with an interdisciplinary team that includes physicians, nurse practitioners, nurses, pharmacists, and behavioral health specialists, you'll help ensure members receive personalized support and a seamless care experience from diagnosis through treatment and beyond. What You'll Do
- Serve as the primary point of contact for members and caregivers throughout their cancer journey
- Build trusted relationships through proactive outreach, support, and engagement
- Help members navigate healthcare services, benefits, claims, and available resources
- Identify and address barriers to care, including transportation, financial, social, and access-related challenges
- Coordinate referrals, intake activities, and member routing to appropriate clinical and support resources
- Educate members on non-clinical aspects of their care and available programs
- Support informed decision-making by helping members understand available services and resources
- Collaborate closely with oncology clinicians, care managers, pharmacists, behavioral health specialists, and other team members
- Respond to member inquiries via phone, email, web-based channels, and other communication methods
- Advocate for members and ensure a positive, personalized healthcare experience
WHAT YOU BRING - REQUIRED

5+ years of overall professional experience
-
3+ years of experience in one or more of the following:• Patient advocacy

Healthcare navigation
-
Care coordination
-
Customer service within healthcare
-
Health coaching
-
Oncology support services
-
Related healthcare roles
-
2+ years of experience working with oncology patients
-
Strong communication, relationship-building, and interpersonal skills
-
Demonstrated care coordination and problem-solving abilities
-
Ability to support individuals facing complex health challenges with empathy and professionalism
-
Experience working within healthcare, payer, provider, or community-based environments
Preferred Qualifications
- Oncology patient navigation experience
- Certified Patient Navigator (CPN) or similar certification
- Experience supporting members with complex or chronic conditions
- Knowledge of oncology treatment pathways and supportive care resources
- Experience working with multidisciplinary healthcare teams
- Bilingual language skills
Education
- High School Diploma or equivalent experience required

Time Type Full time
Pay Range The typical pay range for this role is:
$18.50 - $35.29 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
