Our Lady of Mercy Life Center (OLOM) has a progressive team that embraces innovative technologies like Smart Cell Flooring, designed to help reduce resident falls, and an Artificial Sky that enhances the environment for those in our Memory Care community. In our Sub-Acute Rehabilitation unit, you'll have the opportunity to work with residents recovering from illness, injury, or surgery while developing valuable clinical skills in a supportive, collaborative setting.

We are currently hiring a Full-Time Evening LPN to work 3:00 PM to 11:00 PM, five shifts per week, with every other weekend commitment in our Sub-Acute Rehabilitation unit.

What You'll Do

  • Provide skilled nursing care to residents in our Sub-Acute Rehabilitation unit.
  • Administer medications and treatments in accordance with physician orders and nursing standards.
  • Conduct resident assessments and report changes in condition to the RN Supervisor and healthcare team.
  • Assist in developing, implementing, and updating individualized care plans.
  • Educate residents and families about treatments, recovery goals, and ongoing care needs.
  • Collaborate with interdisciplinary team members to support quality outcomes and successful rehabilitation.

What You'll Need

  • Current New York State Licensed Practical Nurse (LPN) license.
  • Graduate of an accredited Practical Nursing program.
  • Strong clinical assessment and communication skills.
  • Compassion, teamwork, and a commitment to providing exceptional resident-centered care.
  • Long-term care, rehabilitation, or skilled nursing experience is preferred, but new graduates are encouraged to apply.

Pay: $24.00 - $31.10 per hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
