As a Patient Care Coordinator:

• You will develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services available in a market.
• You will primarily conduct facility visits at the physicians' request to assist program clinical team in determining eligibility.
• You will ensure collaboration with program staff and other field sales resources via weekly meetings. Actively participate in weekly program business development meetings.
• You will assist program in processing of physician orders as directed.
• You will report to a Market Executive of Sales

Required/Preferred Experience/Skills:

• RN/LPN/LVN license is required.
• Bachelor's of Science in Nursing is preferred.
• 3 years clinical experience is preferred.
• Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Excellent customer service, time management, problem-solving and communication skills.
• Travel within an assigned territory and to sales meetings as required.
• This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Scheduled Weekly Hours

40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$65,200 - $89,000 per year
