If you are in the McAllen, TX area, including surrounding communities within commuting distance, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Assess, plan, and implement person-centered strategies

  • Connect members with financial, psychosocial, community, and state resources

  • Coordinate care across the continuum and serve as a key point of contact

  • Advocate for members and families to ensure their needs and preferences are heard and supported

  • Collaborate with providers, members, caregivers, and community partners to improve health outcomes

Required Qualifications:

  • Current unrestricted Texas RN license

  • 2+ years of experience in a community health or healthcare setting

  • Intermediate level of proficiency with Microsoft Office applications

  • Access to reliable transportation and valid US driver's license with the ability to travel within assigned territory

Preferred Qualifications:

  • Experience with Medicaid, LTSS, or long-term care services

  • Knowledge of HCBS and integrated care principles

  • Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations

  • Bilingual in English and Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29 - $52 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
