Full job description

Job Duties

HealthPartners is offering a 12-month postgraduate fellowship program to Physician Assistants and Nurse Practitioners interested in advanced training in primary care with a targeted start date of January 2027.

The HealthPartners Primary Care Advanced Practice Clinician (APC) Fellowship is designed to support new graduate APCs in their transition to practice or experienced APCs in their transition to a new specialty. Drawing upon the ACGME Family Medicine Core Competencies and common patient care presentations, the program provides comprehensive clinical and didactic experiences that foster competence and confidence in delivering evidence-based, patient-centered care to in the outpatient setting. Responsibilities include obtaining histories, performing physical examinations, developing differential diagnoses, formulating assessment and management plans, coordinating care, providing patient education and treatment, and participating in all required educational opportunities. Clinical supervision and support are provided by mentoring physicians and APCs.

Primary Care APC fellows will spend a significant portion of their training at an assigned primary care host clinic that is in the Twin Cities or surrounding area with additional rotations in specialty areas that are pertinent to primary care. Candidates will be informed of available host sites during the interview process.

Through diverse rotations, didactic lectures, skills training, and structured mentorship, APC fellows gain the knowledge, skills, and confidence necessary for a successful career in primary care.
