We are seeking a compassionate and skilled Licensed Practical/Vocational Nurse to provide inhome nursing care to patients. In this role, you will support coordinated care with the healthcare team and provide highquality nursing care in each patients home. KEY RESPONSIBILITIES - Deliver skilled nursing care as ordered in the patients plan of care. - Assist in assessing physical, psychosocial, and environmental needs. - Provide patient and caregiver education. - Maintain continuity of care through effective communication and accurate clinical documentation. - Follow infection control and safety procedures. - Participate in case conferences, inservice training, and quality improvement programs. - Uphold agency policies and professional standards. MINIMUM REQUIREMENTS - Graduate of an accredited vocational/practical nursing program. - Active LVN/LPN state license. - Current CPR certification. - Valid drivers license, reliable transportation, and auto liability insurance. - Strong interpersonal, communication, and nursing skills. - Minimum 1 year of clinical nursing experience; home health preferred. WORK ENVIRONMENT - Care provided in patient homes under varying conditions. - Exposure to infectious diseases, blood, and bodily fluids possible. - Requires local travel and ability to work a flexible schedule. - May involve prolonged standing, walking, and lifting up to 50 lbs. *Employees are eligible for different benefits based on their employment status
