As our RN Health Coach in the Clinics, you will be responsible for utilizing the professional nursing process to provide individualized nursing care across the life span in an ambulatory setting. You will care for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up. Additionally, you will serve as a liaison with specialty practices and in-patient facilities, ensuring that patient care plans are updated and coordinated across the continuum.

Everyday you will help patients by working with them and their families using education, goal setting, action planning, problem-solving, and motivational interviewing to support health behavioral change. You will manage disease registries by identifying and following up with patients not meeting clinical goals based on evidence-based Clinical Practice Guidelines. Furthermore, you will assist in conducting pre-visit chart reviews to prepare for comprehensive patient visits by identifying preventive health care needs.

Responsibilities:

  • Facilitate mutual goal setting and goal-directed interactions with the client and family to support health behavioral change.

  • Act as a patient advocate, responding to and working to resolve patient concerns as a liaison with specialty clinics.

  • Participate in quality outcomes analysis and collaborate on monitoring and trending patient responses.

  • Assess medication adherence and identify patients not meeting clinical goals, such as blood pressure or dietary targets.

  • Advocate for disease prevention and healthy lifestyles to reduce inappropriate hospital stays and ED visits.

  • Must hold a valid drivers license.

Preferred:

  • Minimum of three years of professional nursing experience in a clinical care setting preferred
