Schedule: Day shift | Part-time
How you’ll make an impact in this role
-
Drive end-to-end patient care from admission through discharge, prioritizing family values.
-
Monitor progress continuously via clinical assessments, chart reviews, and evaluations.
-
Eliminate care barriers and secure key resources to optimize outcomes.
-
Align healthcare teams and leadership to ensure seamless plan execution.
-
Guide documentation standards, post-acute options, and associate oversight.
What minimum requirements you’ll need
Licensure/Certification/Registration:
-
One or more of the following required:
-
Clinical Counselor obtained prior to hire date or job transfer date.
-
Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
-
Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
-
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
-
Case Manager credentialed from the American Case Management Association (ACMA) preferred.
Education:
- Master's degree in Social Work required.
Work Experience:
- 3 years of experience preferred
