Case Management Analyst

Location: Richmond, VA

Travel: Up to 75%

This Case Management Analyst Field position is with Aetna’s Long-Term Services & Supports (LTSS) team and is a field-based position out of the Richmond, Virginia area. The requirement is for candidates to travel up to 75% of the time to meet with members face to face. This position holds a full caseload to manage waiver members. This position requires in person quarterly visits with members. This position is critical to meeting contractual requirements. Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through the use of care management tools and resources.

Required Qualifications

  • 2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focus

  • Must reside in Richmond Virginia or Surround Areas

  • Must possess reliable transportation and be willing and able to travel up to 70% of the time. Mileage is reimbursed per our company expense reimbursement policy

  • Experience in Microsoft Office Suite

Preferred Qualifications

  • Case management and discharge planning experience preferred

  • Managed Care experience preferred

  • LTSS experience preferred

Education

  • Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling)

  • Certification in LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW preferred

Pay Range

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
