NY1717222 Licensed Practical Nurse (LPN) needed at River Ridge in Amsterdam NY. Full-time positions available. River Ridge Living Center is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our exceptional nursing team. Our 120-bed skilled nursing and rehabilitation facility is committed to delivering high-quality, resident-centered care in a supportive and collaborative environment. Whether you are an experienced LPN or a newly licensed nurse looking to grow your career, we encourage you to apply. RESPONSIBILITIES - Provide direct nursing care in accordance with each resident's individualized care plan. - Accurately assess, monitor, and document residents' health status and report changes promptly to the RN or physician. - Administer prescribed medications, treatments, injections, and other nursing procedures safely and accurately. - Monitor residents for medication effectiveness and potential adverse reactions. - Measure and record vital signs, blood glucose levels, weights, intake/output, and other clinical observations. - Perform wound care and other treatments as directed. - Maintain accurate and timely documentation in the electronic medical record. - Collaborate with physicians, registered nurses, certified nursing assistants, therapists, and other healthcare professionals to provide outstanding resident care. - Educate residents and family members regarding treatments and care plans. - Adhere to all infection prevention, safety, HIPAA, and facility policies. QUALIFICATIONS - Current New York State Licensed Practical Nurse (LPN) license in good standing. - Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or subacute setting is preferred. - Strong clinical assessment and medication administration skills. - Excellent communication, documentation, and interpersonal abilities. - Ability to work independently while collaborating effectively with the interdisciplinary team. - Compassionate, dependable, organized, and resident-focused. - Ability to thrive in a fast-paced healthcare environment.
