Full job description

Responsibilities - Conduct annual nutritional assessments using the nutrition care process. - Design therapeutic diets and menus that meet state guidelines and individual needs. - Provide medical nutrition therapy for individuals served. - Manage tube feeding recommendations and nutritional support needs. - Support the nutritional needs of approximately 90 consumers across 22 group homes. - Assess weight trends and recommend appropriate interventions. - Modify food and liquid consistencies according to NDD and IDDSI standards. - Teach dysphagia in-service training to staff. - Provide clear verbal and written communication to staff, supervisors, and clinical teams. - Fill orders for habilitative equipment to support individual needs. - Provide staff training routinely and as needed.
