Social Work Supports Coordinator

Position Summary To serve as an integral component of VAAA's long-term care services by providing clinical social work expertise in coordinating comprehensive home and community-based services for elderly and other adults, ensuring they can live independently in the community according to their preferences while maintaining strict adherence to VAAA's processes, policies, and professional social work standards. This position combines clinical social work practice with specialized long-term care coordination. Primary Function The Social Worker Supports Coordinator-Long-Term Care Programs informs, assesses, and coordinates a variety of home and community-based services needed by elderly and other adults to live independently in the community as they choose. This position requires clinical social work expertise in conducting comprehensive psychosocial assessments, developing person-centered care plans, and providing case management services within long-term care programs while maintaining adherence to professional social work standards and ethical guidelines.
