Care Manager – Spokane Valley, WA

HomeWell Care Services is looking for a compassionate, organized, and dependable Care Manager to join our team serving the Spokane Valley, Washington area.

As a Care Manager, you’ll play a vital role in ensuring our clients receive exceptional, personalized care. You’ll work closely with clients, families, caregivers, and our office team to coordinate services, monitor care plans, and help create positive outcomes for the people we serve.

Pay Range

  • $26 - $30/ hr. D.O.E.

What You’ll Do

  • Build trusting relationships with clients and their families

  • Conduct client visits and assess changing care needs

  • Develop and monitor individualized care plans

  • Coordinate schedules and communicate with caregivers

  • Respond to client and family concerns with professionalism and empathy

  • Monitor service quality and client satisfaction

  • Support caregivers and help ensure consistent, high-quality care

  • Maintain accurate documentation and communicate important updates to the team

What We’re Looking For

  • Compassionate and client-focused approach

  • Strong communication and organizational skills

  • Ability to manage multiple priorities independently

  • Experience in home care, caregiving, care coordination, healthcare, or a related field preferred

  • Reliable transportation and ability to travel locally as needed

  • Professional, positive attitude and commitment to excellent service
