  • Lead physician input to a focused portfolio of interventions in assigned domains, with a clear clinical rationale and measurable goals
  • Address differences in access, experience, and outcomes when designing programs
  • Define eligibility, workflow, accountable owner, expected effect, leading measures, outcomes, and scale or stop criteria for each intervention
  • Partner with Analytics, Finance, and Actuarial to evaluate clinical and economic results and explain limits of the evidence
  • Change or retire programs that are ineffective or create excessive burden
  • Design workflows with the CCO, SVP Population Health, market leaders, and front-line teams so programs fit home-based care
  • Equip clinicians with practical guidance, registries, decision support, education, and escalation paths
  • Monitor reach, adoption, fidelity, capacity, clinician burden, patient impact, and variation across markets
  • Frame the highest-value clinical questions for Analytics and turn findings into clinical decisions
  • Guide responsible use of predictive and AI tools, including calibration, bias, human oversight, and workflow fit
  • Represent assigned clinical programs with executives and external partners when appropriate
  • Employees will be required to perform other job-related assignments as requested

Qualifications Required Knowledge, Skills, and Experience

  • Demonstrated ability to design, implement, and evaluate clinical interventions across a distributed organization
  • Strong fluency in clinical epidemiology, risk stratification, quality measurement, utilization, and total cost of care
  • Ability to work closely with analytics, actuarial, finance, technology, and operations teams and to explain limitations without losing decision velocity
  • Track record of influencing field clinicians and senior executives without relying solely on formal authority
  • Formal training in public health, epidemiology, health services research, informatics, quality, or business preferred

Preferred Knowledge, Skills and Experience

  • Prior experience with frailty, serious illness, home-based care, geriatrics, transitions, pharmacy, behavioral health, or social care
  • Prior experience in ACO REACH, MSSP, Medicare Advantage, or similar risk arrangements and in pragmatic program evaluation
