Hospice Admissions Nurse
Full-Time

As an Admission Nurse , you’ll contribute to our success in the following ways:

Assess hospice patients to identify physical, psychosocial, environmental, and nursing needs.

Complete initial evaluations and reassess patient needs and condition during each visit.

Provide care according to the individualized Plan of Care to support quality, continuity of care, and positive patient outcomes.

Monitor for changes in patient condition and adjust the Plan of Care or take appropriate action as needed.

Communicate significant changes, concerns, or safety issues to the physician, Patient Care Manager, and other members of the care team.

Maintain accurate clinical documentation while protecting patient privacy and confidentiality in accordance with HIPAA requirements.
What is Required?
-
Current, unrestricted state RN licensee
-
Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
-
1+ year experience in a clinical care setting as a nurse
-
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
-
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
-
Associates Degree or Bachelors Degree in Nursing is preferred
-
Previous Home Health or Hospice Experience is preferred
-
Previous experience with HomecareHomeBase is preferred
