Social Care Navigator

POSITION SUMMARY The Social Care Navigator II (SCN) connects individuals and families with essential health and socialservices. Operating primarily in the field alongside in-person and telephonic outreach, the SCNassesses health-related social needs (HRSN), facilitates resource referrals, and manages caseloadsdirectly within community settings to ensure clients receive comprehensive supportincluding foodpantry and grocery distribution for eligible members. KEY RESPONSIBILITIES SCN Screening & Navigation Conduct direct field outreach and engagement with Medicaid members in community settings, aswell as telephonically, to assess health-related social needs (HRSN).Perform comprehensive field-based screenings and manage referrals to appropriate healthcareproviders, social service agencies, and local community resources.Document client eligibility, field outreach activities, referral outcomes, and case notes accuratelyusing designated tech platforms in compliance with established protocols.Track referral progress and follow up in the field to ensure client needs are met effectively.Collaborate with internal team members, community partners, and partner-based CHWs/navigators on-site and across community organizations to support clients with complex needs.Perform other tasks as assigned by the Senior Director of NYREACH. Food Insecurity & Pantry Services Manage an assigned client caseload, providing enhanced support services and direct distributionof pantry/grocery boxes to eligible members in the community.Monitor and manage service referrals received through the Findhelp portal, delivering internalservices or referring members to appropriate external programs.Partner directly with community food pantries and local organizations to coordinate donations,establish referral pathways, and align services. QUALIFICATIONS & REQUIREMENTS Education: Bachelors degree in Social Services, Psychology, Public Health, or arelated field.Languages Bilingual proficiency preferred (Spanish preferred).Core Skills: Strong verbal and written communication, active listening, field navigation,and organizational abilities.Competencies: High cultural sensitivity with the ability to build rapport and work effectivelywith diverse populations in community environments.Experience: Background in field-based social services, case navigation, or communityoutreach preferred. WORK ENVIRONMENT Field-Based Position: Majority of time is spent conducting direct community outreach, visitinghealthcare facilities, partner organizations, and local community events, complemented by officetime at 2024 Creston Ave. Interaction: Direct, in-person client engagement in field and community settings, supplemented byphone and electronic communication.
